I used to have obsessive fears of death by fire and death by car crash, I hope to leave the world a better place than I found it.

I want it to be peaceful and without pain, preferably in my sleep.
It's not for me, it's for those who will have to deal with my death. I won't care one way or the other. :)

I opted for conscious, so I suppose laughing works pretty cool too.
It'd just be a drag to be out of it and miss the most significant moment of my existence since birth.
Actually I almost didn't clear that hurdle.
I was a feet first breach and was being strangled by the umbilical cord.
Needless to say Doc Hohnan got me out so I'm glad it wasn't a rattle shaking new-age midwife who would have sacrificed a chicken although there wasn't much woo-woo new age stuff around in January of 1955 but the sixties were just around the corner.
BTW it's not a knock on responsible midwives just on some of the flakes I've come across.

Dragged, kicking and screaming, and holding on by my fingernails to every last precious second of life.
I want the hospital bed, the tubes, the machines doing everything for me until machines is pretty much all that's left. I want my life prolonged by as much as medical science can possibly manage and then prolonged some more.

Leaving the world a better place, of course. To have succeeded. Then, when that's over with, I want to fall from orbit.I'd like to die from falling through the atmosphere (that is, if I absolutely have to die and haven't figured out a way to stop my demise by then).
Death by fiery disintegration. Ultimate rush.

I have been connected to the military in one form or another for over 25 years.
Being a warrior, I would like to go out fighting!!!
Hopefully I would go out saving lives.
Something like the scenario in the movie "Star Trek Nemesis" where Data saves the Starship Enterprise and its crew.

As long as I didnt die alone I'd be happy. If I couldnt at least die with true friends by my side, then that would be really uncool. As long as I got to say goodbye.

It isn't really that big a deal, but not enduring a lot of pain would be a plus.
I had asthma as a young kid, thankfully it has not been an issue since high school, but I think suffocation would be the worst for me. I remember what it was like to have trouble breathing.
I also had a heart attack last October, which an angioplasty and stent repaired, so that would not be a nice trip to do again. Yes, I am taking the necessary steps to prevent, or otherwise delay a repeat performance as long as possible.
Be that as it may, unless one contemplates the idiocy of suicide, one seldom has a choice. I want to live until I die, and all I desire after that is to be able to see death coming, look it in the eye and say "PISS OFF!!!":p

Um, I don't want to leave at all. I know I will eventually, but there is no particular way I want to go. I'll try to live the best way I can, but I don't care in what way I die, as long as it's not soon.
I would have voted for the second to last if it didn't have the word 'happy' in it.

If I'm healthy and handsome enough at 100 to pull a young girl I wouldn't want to die just yet.

Depends on how you define 'heat death'. If you allow it to include living beings and electric blankets you have dimished it greatly.
As long as we're on this subject, I want to survive the end of this universe, either by escaping to another, or by reversing entropy, or both.

Most likely at one hundred, one would be arrested for assault with a dead weapon
