I always think of the Mafia when I read something like this.

And then I think "ok, it was probably organized crime of some kind, but not necessarily the mafia". Biker gangs or urban gangs fighting over turf or something, maybe?

My first thought was hospital waste. A friend of mine used to work in a hospital laundry and it was absolutely amazing what used to make it out of the operating theatre (so much for careful instrument counts).
