Quite a few performers have gashed or impaled themselves accidentally doing similar feats. I believe Chris Korn did so on one of his television shows.

Most of the performers were simply careless.

If memory serves, David Copperfield was careless during one of his performances, cutting off part of one of his fingers with a scissors. Like Hultgren, Copperfield stayed calm and informed the audience that he had hurt himself, and he promptly sought medical attention.

I've seen a few YouTube (or sumesuch) videos of performers screwing up the very same trick in the same way. In one of them, it is the audience member that the magician has slamming their hands down on the cups.

They were all brutal to watch, especially since you knew what was coming.

Oh man, Malaka, I watched that and it was the most cringe-worthy piece of video I've ever seen in my life.

I've seen a video of the spectator getting impaled,and boy was she angry!
Dunno what kind of lawsuit followed-it was in the U.K.
